Setting Your Health Goals
On or before the first visit, new patients complete intake paperwork that includes medical history and a section where you list three goals for your care. Pain reduction is a popular choice as one of the goals, but there are often additional results people desire.
The paperwork is sent in a confirmation email before your first scheduled appointment. You can either fill everything out before coming in, or you can arrive at our office about 15 minutes early and complete the paperwork in-person.
Once goals are determined and a complete spinal analysis has been completed, you and Dr. Matt will set a course of care and action that leads to the intended outcome. The care plan is usually set on your second appointment, along with any supplements and any exercises you can start doing at home.

On both the first and second visit, Dr. Matt will lead a discussion with you on the financial options for covering your chiropractic care. We try to speak in general on the first visit, while on the second visit we go more in-depth on the details.
ProHealth Chiropractic is in-network with most major insurance providers. Health savings accounts and all major credit cards are acceptable forms of payment.
